There is no Western-style hospital care in Thailand. For foreigners, tourists or expat residents, you will only receive treatment if the hospital accepts that your insurer will pay or you have sufficient funds on your credit card. No pay, no treatment.
For those over 70 who cannot get insurance cover that means paying cash up front. And costs can be around two or three times more than that charged to Thai citizens. In fact, most Thai nationals receive free care.
